Holt Jr Rams CASL Registration - Fall 2016 Here are the steps to complete CASL registration for your child:

REGISTRATION NIGHTS
Tuesday, July 26th 6:00-7:30pm
Wednesday, August 3rd 6:00-7:30
Kiwanis Park, under the pavilion by the softball field
Directions: Right by the corner of Maple St. & Lee St., Holt, MI 48842
From Sycamore St. go south on Maple until it ends at the parking lot for Kiwanis Park

Complete the following forms and turn them in to your at one of the registration meetings

MSYSA Medical Release Form (must be notarized)
Parent/Athlete Concussion Information Sheet
Family Contact Information Form
Uniform Order Form DUE AUGUST 12th to Johnny Mac's (REQUIRED ITEMS: Black jersey, White jersey, shorts & socks)
Email a digital photo (headshot) of your child to your coach (only used for online player check-in system at games)
$150 Check payable to HDSO or cash
DEADLINE to apply for Financial Aid is August 1st. Go to: http://caslsoccer.org/forms/financialassistance to apply

Most teams begin practicing around the 3rd week of August
First games of the Fall season will begin after Labor Day
